The Care Partner plays a vital role in providing high quality, relationship-based, person-centred in-home care, tailored to each individual’s needs, goals and preferences. The Care Partner builds trust and rapport with their participants and supporters by working collaboratively to gain a deep insight into age-related health needs within the context of the participant’s life. This relationship-based support and planning underpins every care management activity completed by the Care Partner, driving improved outcomes, continuity of care and participant satisfaction. This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with a starting annual salary of $88,090 + Superannuation + Salary Packaging.
