Key Responsibilities Manage a client case load planning and review client goals to ensure high-quality, person-centred care. Communicate proactively with clients, families, and team members to develop and update care plans. Meet with clients and families face-to-face to review care plans and ensure needs are being met. Accurately document all client interactions and share relevant information with the team. About you Previous and relevant experience working with seniors or aged care. Background in care coordination, case management, or client-facing support roles Experience building relationships with clients and families, preferably in a home care or community setting. Able to work under pressure and multitask shifting priorities. Desirable: qualification in a health-related field such as nursing, allied health, social work, or community services.
